An exciting opportunity has arisen to employ a Discharge Liaison Nurse to work with the North Glasgow Palliative Care Fast Track Discharge Service based at the Marie Curie Hospice Glasgow.
! This is a 25 hour post working over 5 days and will be a fixed term contract till 31ST March 2015.
This service enables the safe and timely discharge of patients with palliative or end of life care needs from Glasgow Royal Infirmary or Marie Curie Hospice Glasgow. This service compliments existing community services including these provided by the District Nursing Teams, Cordia and overnight Marie Curie Nursing Service.
As a Discharge Liaison Nurse you will asses the care needs of end of life patients who wish to be cared for in their own home and facilitate a timely discharge. You will work as part of a larger team of Discharge Liaison Nurses and Senior Health & personal Care assistants.

You should have experience of care planning and providing care for patients and families with Palliative care nee! ds in the community, along with a driving licence and access t! o a car.
This post will be offered as a Marie Curie post as well as a Secondment opportunity for existing District Nurses working in Glasgow. If you are considering a secondment please discuss with your line manager before applying for the post.
