Mount Carmel Community Hospital is a Transitional Care Unit. This unit provides ongoing medical monitoring, rehabilitation or re-enablement to patients following an acute hospital stay. Mount Carmel Community Hospital is there to help patients with their expressed desire to make a speedy recovery and return home.
Mount Carmel is now seeking an experienced Social Worker to join their team. The successful candidate will be required to have regular and effective liaison with the discharge teams in the acute hospitals, as well as the community care teams. The Social Worker will support individuals with activities of daily living and enable them to live independent lives in line with HIQA national standards

The Social Worker will be an integral member of the Mount Carmel Patient-Flow Team and will actively participate in Multidisciplinary team meetings.

- A third level qualification in Social Care or relevant field.
- Registration with CORU
- Experience of working in the acute hospital setting in relation to safe and effective patient discharge planning
- A good understanding of the healthcare and social support systems in the community and how to access care packages for patients requiring home care support following their discharge.
- Experience of working in the Community
- Ability to work effectively as part of a multidisciplinary team
- Experience maintaining relationships with stakeholders in the provision of health & social care services.
- Experience in the support of independent living skills and community integration.
