Salary
£30,537 - £32,436 36 hours per week Maternity Cover for 12 months
Job Requirements
Profe! ssionally qualified in Social Work, Occupational Therapy or Nursing you will have an assessment and care management role within a dynamic and proactive multi-disciplinary social care team.
Your work will be challenging and provide opportunity for creative person centred practice. Applicants should be experienced and have knowledge of working across the Community Care spectrum and possess the skills to achieve a safe balance of promoting independence and managing risk.
You must be an excellent communicator, working as part of a team who can develop relationships with colleagues and service users. You will show a commitment to an outcomes focussed approach within your working practice.
This is a temporary post to cover maternity leave.
