The LAC Health Team aims to assess, identify and promote the health and well being of those children and young people who are looked after by the three local authorities of ForthValley. This p! rocess involves the gathering, collating and assessing of health information about children and young people referred for a comprehensive health assessment and, in conjunction with them, plan / implement and evaluate a programme of health improvement. This will be evidence based, utilising all available resources, taking into consideration the lifestyle, gender and cultural background of each child and ensure involvement with partner agencies and families. The Throughcare/ Aftercare PHN would support young people to identify and manage their own health needs once they have left the care of the local authority by offering health needs assessments, r eviews and advice on existing health problems, advice on lifestyle issues, promoting healthy living and supporting young people to access appropriate health services.
The post holder will work within The LAC Health Team, based at StirlingCommunityHospital and will report to The Team Leader for LAC for professional manage! ment, work review and formal appraisal of performance. The pos! t holder is required to work autonomously whilst ensuring strong links exist with the team and all associated agencies, statutory and non-statutory, involved in supporting the young person.
