POSITION: CARE PRACTITIONER

GENERAL RESPONSIBILITIES

1. To provide the highest standard of care to all residents within Marchmont Homes Limited
2. To work efficiently as part of a team, respecting all of your colleagues
3. To adhere to all tasks and duties given to you by the Senior Care Practitioners , or the management
4. To read, understand and adhere to all policies operational within Marchmont Homes Limited

RESPONSIBILITIES IN MORE DETAIL

1. To provide the highest standard of care to all residents within Marchmont Homes Limited

  • Always be aware of each resident’s individuality, dignity, personal needs and preferences
  • Be communicative and friendly to all residents, r! emembering that when you are at work you are in their home
  • Support and aid residents who need assistance with getting up, general mobility and sitting down
  • Support and aid residents who need assistance with washing, toileting, dressing and undressing
  • Where there are mechanical and non-mechanical aids, always ensure that they are being used in the correct and most efficient manner by both yourself, your colleagues and the residents themselves
  • Always be aware of each individual resident’s medical, physical, mental or other disabilities or constraints, and take them in to consideration when supporting, communicating or generally caring for that individual
  • Always be aware when an individual resident has a progressive or terminal illness
  • Consult each individual resident’s care plan
  • Maintain each individual resident’s care plan when and where it is necessary to do so
  • Ensure that all resident’s are taking the appropriate fluid levels according to best practice an! d their individual care plan
  • Ensure that all resident’s are taking the appropriate food levels according to best practice and their individual care plan
  • Assist residents with eating and drinking where it is necessary and/or appropriate
  • Regularly check resident’s according to best practice and their individual care plan
  • Answer the Nurse Call system as efficiently as possible when there is a need to do so
  • When and where appropriate, encourage residents to join in activities and communicate with other residents
  • Make and change bed linen when and where required and ensure that resident’s rooms are clean and tidy, that commodes are emptied regularly, clothes are distributed to the in-house laundry facility when required and that the resident has things as they like them in their room
  • Clean and maintain all equipment used by the residents and/or their visitors, when and where required to do so
  • Immediate! ly report any illness, accident or incident incurred by a resident to t! he Senior Care Practitioner in charge of the shift or the Manager
  • Where there is uncertainty on any of these points consult the Senior Care Practitioner on duty
  • Always treat resident’s relatives and other visitors with respect and be welcoming to them, offering them refreshments and (when appropriate) advice on the well-being of the residents they have come to see
2. To work efficiently as part of a team, respecting all of your colleagues

  • Be punctual at all times when required on duty
  • When you are unable to attend work, be aware of the effect your absence has on your colleagues and the residents and ensure that you give as much notice as possible to those in charge of the rota, in order that they can arrange cover
  • Be aware of the example you are setting to other members of the team as well as the impression you are leaving on residents and their visitors at all times
  • Maintain a high standard of work! eth ic to promote the good name of Marchmont Homes Limited
  • As and when there is no obvious task to be done, assist your colleagues and expect the same of them
  • Be communicative and respectful of work colleagues when in the work place
  • When giving or taking instruction always do so calmly and collectively, especially concerning the direct assistance of a resident
  • Any grievance concerning another member of staff should be reported to the S enior Care Practitioner , Manager or (if necessary) the proprietor at an appropriate time and in an appropriate manner
  • Always be aware that they way you respond to your colleagues in turn reflects the way that both your colleagues and the residents respond to you
3. To adhere to all tasks and duties given to you by the Senior Care Practitioners , or the management

  • When taking instruction always do so calmly and collectively, especially concerning the direct assistance ! of a resident
4. To read, understand and adhere to all polic! ies operational within Marchmont Homes Limited

  • Ensure that you have read and understood and that you adhere to the health and safety, fire and safety, waste disposal and all procedural and other policies that are operational within the home
  • Always ensure you maintain complete confidentiality of all matters concerning the residents, your colleagues or the business , this also includes social websites
  • Always ensure that the security of the Home is maintained at all times
  • Attend and participate any meeting or training sessions that you are required to attend
  • Be aware of any changes to any policies within the home and ensure that you have read through and checked them as soon as possible after any change
  • Always report any grievances, faults or potential hazards to the Manager
As per the terms and conditions of employment, this job description is not exclusive and, although it does indicate the main elemen! ts of the job, there may be other duties, which from time to time, a Care Practitioner may be required to undertake in order to meet the requirements of the management of Marchmont Homes Limited.
